Hospice Care at Home
Caregiver and family members often worry about their ability to provide comfort for a seriously ill loved one at home. Homestead’s expert support team provides guidance and education of disease-specific symptom management with a unique anticipatory Homestead Symptom Pathway for Crisis Management. Through the disease-specific Crisis Pathway, the caregiver will have an enhanced awareness of symptom management and more confidence in caring for their loved one at home. Families feel a sense of relief and satisfaction knowing they can keep their loved one comfortable until a Homestead team member arrives. The Homestead support team is only a call away to provide that extra level of support and peace of mind.
Hospice Care in a Facility
If a patient calls home a nursing center or assisted living facility, and their physician has determined they have six months or less to live, Homestead will coordinate hospice services with the contracted facility. Understanding and acknowledging the importance of communication and care collaboration between family members and facility staff is key to hospice care. Homestead’s professional team will communicate directly with the family and facility to keep everyone updated on the patient's status after each visit.
Inpatient Care
If the patient and family has any reason to have their loved one in an inpatient hospice unit, Homestead will provide the best care possible in a contracted facility to ensure all comfort measures.
